Informed Consent and Membership Terms By joining, you acknowledge that our services are wellness services, not medical treatment, and results vary by individual. Consult your healthcare provider if you have medical conditions, are pregnant, or have device-specific contraindications. Follow all posted room rules and staff instructions. Billing and renewals: First full month is due at sign-up. Membership auto-renews monthly on the same calendar day. Sign-ups that occur on the 29th, 30th, or 31st will renew on the 28th. You may freeze or cancel any time after the initial month. Freezes and cancellations: Fastest method: submit online at spraysunspa.com for same-day processing. In person: we provide a signed receipt by email, remaining prepaid time may be used. Phone: processed on a recorded line, you receive the associate name and a unique code word used as your signature reference. Text or email: must come from a verified number or email, you receive a reference number. Voicemail requests are not accepted. If you do not receive written confirmation, the membership remains active. Portal freezes default to one month. Upgrades or downgrades must be done in store. Records: Keep all receipts and confirmations. If a dispute arises and you cannot provide proof, the original agreement and billing will be considered valid. Payment methods and fees: A backup card on file is required to avoid a 15 dollar decline fee if the primary card declines. Balances may be paid with cash or an alternate card. Pricing includes a 15 dollar pay-on-time discount each month. If payment is not received by auto-draft or prepay, that month’s balance increases by 15 dollars. Payments are due regardless of usage and continue until you submit a cancellation through an accepted channel. If a draft is declined, we may attempt daily redrafts for the past-due amount and applicable fees. The decline fee is automatically waived if the balance is paid the same day via the text link or in store. Card and identity: The card on file must match the name on a photo ID at your first visit. The card used at sign-up will be charged each month unless you update it in person or in the account portal. Term acknowledgement: This membership is an open-ended agreement and will continue until a freeze or cancellation is properly processed. Additional rules and restrictions may apply. Full terms of use are provided and signed at the salon.

Cancellations handled online at www.spraysunspa.com are cancelled same day, cancels done in person will have signature proof emailed to member's email with date and remaining prepaid time can be used, cancellations handled over the phone will be done over a recorded line and member will be given the associate name and code word that will be signed on their behalf on the cancel agreement to reference, text cancellations will only be allowed from verified text number and will also be given a reference number, same with email cancellation requests. No voicemail cancellation requests will be accepted and do not assume membership is cancelled if you did not hear back from us. Membership changes must be done in store like upgrading or downgrading membership, freezes may be in all ways similiar to cancel, if using the account portal it will only freeze one month. Reciepts and agreements must be retained by ciustomer in case of a discrepency, without client having proof the original contract will be assumed valid. Account will require a back up card on file to avoid decline fee of $15 if primary declines. Balances may be paid with cash or alternate card. Price listed includes a $15 pay on time discount applied each month, if payment is not recieved with auto-draft or prepay the balance will increase by $15 for that month, payments are due regardless of use and continue until cancellation is submitted online and SSTT will continue to attempt redrafts. Decline fee is automatically waived if balance is paid same day as decline through text link sent notifiying of decline or paying in store. Redrafts occur daily to satisfy backowed amounts and will include decline fees. -- OPEN-ENDED AGREEMENT -- auto-renewal will continue until proper cancellation is processed, other rules and restrictions may apply.
